CHANGES TO THE REQUIREMENTS FOR SUBMISSION OF PPEs:

There has been a change in the requirements for submission of the updated PPE. The History Form and the Physical Examination Form within the packet do not need to be submitted to the student’s school. The History Form and the Physical Examination Form should be kept by the healthcare provider who completes the PPE. Only the completed Medical Eligibility Form (MEF) should be submitted to the school. The Medical Eligibility Form provides space for the healthcare provider to share relevant health information with the school if necessary and includes a recommendation regarding the student’s ability to participate in athletics. It is important to note that the MEF must be conducted by a licensed physician, advanced practice nurse, or physician assistant who has completed the student athlete cardiac assessment professional development module developed by the NJDOE. 

IN ORDER TO BE ELIGIBLE FOR TRYOUTS, A STUDENT MUST:

  • Have had a physical within 365 days prior to the tryout date.

  • Submit a parent and physician completed NJ state mandated Medical Eligibility form to the health office for clearance by our school physician, Dr. Porter.

  • Sports forms must be submitted to the Health Office by Friday, March 7.  All Medical Eligibility forms will be reviewed by me and cleared for try-outs by the school physician before the Spring season. Tryouts are tentatively scheduled for March 19-21.

  • Students who do not have the mandated MEF turned in (and Health History Update form, if needed) WILL NOT BE ELIGIBLE FOR TRYOUTS!

  • Dr. Porter will be in to clear all MEFs on Thursday, March 13.

PLEASE NOTE:

  • A 'Health History Update' form ONLY needs to be completed for students whose MEF is dated greater than 90 days from the first day of try-outs of any sports season.

  • NEW FOR 2024-25: The 'Sudden Cardiac Death in Young Athletes', 'Opioid Use and Misuse,' 'Opioid Video Memo and Sign Off Sheet,' and 'Sports-Related Concussion and Head Injury' forms WILL BE SIGNED IN THE PARENT PORTAL under the Forms tab. These are mandated forms and will be considered current for the entire school year.

  • Pursuant to N.J.A.C. 6A:16-2.2, written notification signed by the school physician stating approval/disapproval of the student's participation in athletics based upon the MEF will be sent home, noting the date of the MEF for your records.

  • It is the parent's and student's responsibility to contact me to ensure all required forms are on file:

    • It is too disruptive to academics for me to locate students to get them any forms they may need. 

    • State forms can be printed out from this web page or picked up in the Health Office.
